We take a personalized approach to the unique needs and challenges faced by those we work with ranging from identifying suitable affordable housing options, assistance with rent to navigating rental agreements and beyond.

Eligibility
YOU MAY QUALIFY TO RECEIVE HOUSING STABILIZATION SERVICES IF YOU:
Have Medical Assistance (MA) coverage
Are 18 years or older
Have a disability or disabling condition: disabilities can include physical disabilities, mental illness, substance use disorder, and other conditions.
Are in one of the following situations: experiencing homelessness, at risk of homelessness, moving out of (or moved out of in the last six months) an institution or at risk of institutionalization
Require assistance with at least one of these disability-related areas: communication, mobility, decision-making, or managing moods/ behaviors
